Xylitol and Oral Health: What the Evidence Says

Xylitol is a sugar alcohol that resists fermentation by oral bacteria, giving it a plausible mechanistic advantage over sugar. But when systematic reviews actually pool the clinical trial data, the caries-preventive effect of xylitol has not been conclusively confirmed — the evidence remains genuinely mixed, not simply "not yet enough studies."

What Xylitol Is

Xylitol is a naturally occurring sugar alcohol found in small amounts in fruits and vegetables and produced commercially from sources like corn cobs or birch. It tastes about as sweet as sugar but has a lower glycemic impact and, importantly for oral health, is not readily metabolized by Streptococcus mutans and other acid-producing oral bacteria the way sucrose is.

Why the Evidence Is Inconsistent

Reviewers repeatedly flag the same issues: studies use different xylitol doses (many below the amount associated with the strongest historical results), different delivery formats (gum, lozenges, tablets — as in ProvaDent), different durations, and different populations. It's genuinely difficult to isolate xylitol's independent effect from the simple fact that chewing gum stimulates saliva flow, and that participants in a dental trial often brush and floss more diligently than usual.
